Welcome

Hello and welcome to the Community Link, Inc. (CLI) website. My name is Mark Hilliker and I have the great honor of serving as CLI’s Chief Executive Officer.

 

Community Link, Inc. (CLI) was formed on January 1, 2017 to leverage 48 collective years of experience in delivering a person-centered and community-focused approach to the Family Care program across a large portion of Wisconsin. While the name may be new, CLI is not new to long-term care. In fact, its founding organizations—Community Care Connections of Wisconsin, Western Wisconsin Cares, and ContinuUs—have been industry leaders for nearly two decades. In operation since 2000 as county pilot participants, CLI’s rich history and collective experience is time-tested, refined, and has consistently performed at the high end of MCO metrics regarding quality, cost efficiencies, and member satisfaction. With proven success in multiple expansions into new Family Care counties and service regions, robust network development, and collaborative stakeholder engagement, CLI is committed to providing continued excellence in the delivery of long-term care supports and services across Wisconsin.

 

What makes the merger of the three founding organizations into Community Link, Inc. so exciting is its expansive impact. Our reach will span 2/3 of Wisconsin, but more importantly, our support will enhance hundreds of neighborhoods and communities, and bring together thousands of citizens. Our work centers on the common good for all, and this merger grows that collective commitment substantially.

 

CLI is an organization with over 1,100 dedicated employees. We contract with nearly 4,000 long-term care services and supports providers across the state. We partner with a variety of other people, organizations, agencies, and local governments to provide Wisconsin’s Medicaid managed care program called Family Care.

 

We are grateful to have the opportunity to support over 15,000 adults with disabilities and elders across 46 Wisconsin counties. At CLI, we look at ourselves as partners in all that we do, with special emphasis on adding value to every relationship we enter into. We are focused on building inclusive and dynamic communities where everyone’s gifts and talents are recognized and valued, and where every person can live a meaningful life, connected to those things most important.
