Community Link, Inc. (CLI) is coming to a neighborhood near you! Our goal is to partner with you as we transition Family Care into Forest, Vilas, Oneida, Florence, Taylor, Adams, Ozaukee, Sheboygan, and Walworth, Counties. To support that transition, we’ve put together resources, contracting next steps, and contact information as we navigate this partnership together.

Our organization, CLI, manages the provision of long-term care services and supports for over 15,000 Family Care members across a forty-six (46) county service region. We currently employ 1100 people across 30 office locations throughout Wisconsin and manage over 3,400 contracts with direct service providers within nearly 40 service categories. Our intent is to provide exceptional continuity of care for the individuals in your county. We pride ourselves on hiring local and being a part of the communities that we serve.

Contracting and Next Steps

We look forward to partnering with you. There are a few steps to become a contracted provider in the CLI network. To start, please complete the following:

Application Form
W9 Form (for EIN or Social Security Number use the tax number you use for claims filing)
Proof of insurance
Proof of License and/or Certification (if applicable)
Residential Provider Services Form (for residential providers only)
